There are three simple steps to get your rebate:
• Purchase any new Blessing ML1 trumpet from an authorized Blessing dealer in the USA between April 1, 2011 and June 30, 2011. Save your receipt!
• Mail a copy of the original store receipt (not a credit card receipt, please), proof of serial number by enclosing the UPC code from the box or from the plastic wrapping around the instrument, and the completed form below by no later than July 31, 2011 to:
